ABSTRACT: Intravoxel incoherent motion (IVIM) diffusion-weighted magnetic resonance imaging (MRI) can be used to estimate perfusion-related parameters, but these parameters may differ, based on the curve-fitting algorithm used for IVIM. Microvessel density (MVD) and vascular endothelial growth factor (VEGF) status are used as angiogenic factors in breast cancer. We aimed to investigate the relationship between MVD, VEGF, and intravoxel incoherent motion (IVIM)-derived parameters, obtained by 4 curve-fitting algorithms, in patients with invasive breast cancers.This retrospective study investigated IVIM-derived parameters, D (ie, tissue diffusivity), D∗ (ie, pseudodiffusivity), and f (ie, perfusion fraction), of 55 breast cancers, using 10 b values (range, 0-800 s/mm2) and 4 curve-fitting algorithms: algorithm 1, linear fitting of D and f first, followed by D∗; algorithm 2, linear fitting of D and f and nonlinear fitting of D∗; algorithm 3, linear fitting of D and f, linear fitting of D∗, and ignoring D contribution for low b values; and algorithm 4, full nonlinear fitting of D, f, and D∗. We evaluated whole-tumor histograms of D, f, and D∗ for their association with MVD and VEGF.D∗10, D∗25, D∗50, D∗mean, D∗75, D∗90, f10, and f25, derived using algorithm 3, were associated with VEGF expression (P = .043, P = 0.012, P = .019, P = .024, P = .044, P = .041, P = .010, and P = .005, respectively). However, no correlation existed between MVD and IVIM-derived parameters.Perfusion-related IVIM parameters obtained by curve-fitting algorithm 3 may reflect VEGF expression.

BACKGROUND: In diffusion-weighted imaging (DWI) of breast MRI, simultaneous multislice acceleration techniques can be used for readout-segmented echo planar imaging (rs-EPI) to shorten the scan time. PURPOSE: To compare the image quality, apparent diffusion coefficient (ADC) value, and scan time of rs-EPI and simultaneous multislice rs-EPI (SMS rs-EPI) sequences. STUDY TYPE: Retrospective. SUBJECTS: In all, 134 consecutive women (mean age: 55.3 years) with invasive breast cancer who underwent preoperative MRI. FIELD STRENGTH/ SEQUENCES: 3.0T; rs-EPI sequence, prototypic SMS rs-EPI sequence and dynamic contrast-enhanced MRI (DCE-MRI) sequence ASSESSMENT: For quantitative comparison, two radiologists independently measured the signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR), lesion contrast, and apparent diffusion coefficient (ADC). For qualitative comparison, image quality, lesion conspicuity, and reader preference were assessed with a reference of DCE-MRI. STATISTICAL TESTS: Paired t-tests and Mann-Whitney tests were used. RESULTS: For SNR and CNR, there were no differences between the sequences (P = 0.342 and 0.665 for reader 1; P = 0.606 and P = 0.116 for reader 2). Lesion contrast of SMS rs-EPI was higher than that of rs-EPI (P < 0.05 for both reader 1 and reader 2). Mean tumor ADC was similar in rs-EPI and SMS rs-EPI sequences (0.98 ± 0.22 vs. 1.00 ± 0.22; P = 0.291 for reader 1, 0.98 ± 0.21 vs. 1.00 ± 0.22; P = 0.418 for reader 2). Regarding qualitative comparison, image quality and lesion conspicuity were higher in SMS rs-EPI than in rs-EPI (both P < 0.05 for both readers). The two readers regarded SMS rs-EPI as superior or equal to rs-EPI in over 90% of cases. The acquisition time was 4:30 minutes for rs-EPI and 2:31 minutes for SMS rs-EPI. DATA CONCLUSION: The SMS rs-EPI sequence resulted in a similar ADC value and better image quality than the rs-EPI sequence in a 44.1% reduced scan time. LEVEL OF EVIDENCE: 4 TECHNICAL EFFICACY: 3.

OBJECTIVES: To investigate the value of the whole-lesion histogram apparent diffusion coefficient (ADC) metrics for differentiating low-risk from non-low-risk ductal carcinoma in situ (DCIS). METHODS: The authors identified 93 women with pure DCIS who had undergone preoperative MR imaging and diffusion-weighted imaging from 2013 to 2016. Histogram analysis of pixel-based ADC data of the whole tumour volume was performed by two radiologists using a software tool. The results were compared between low-risk and non-low-risk DCIS. Associations between quantitative ADC metrics and low-risk DCIS were evaluated by receiver operating characteristics (ROC) curve and logistic regression analyses. RESULTS: In whole-lesion histogram analysis, mean ADC and 5th, 50th and 95th percentiles of ADC were significantly different between low-risk and non-low-risk DCIS (1.522, 1.207, 1.536 and 1.854 × 10-3 mm2/s versus 1.270, 0.917, 1.261 and 1.657 × 10-3 mm2/s, respectively; p = .004, p = .003, p = .004 and p = .024, respectively). ROC curve analysis for differentiating low-risk DCIS revealed that 5th percentile ADC yielded the largest area under the curve (0.786) among the metrics of whole-lesion histogram, and the optimal cut-off point was 1.078 × 10-3 mm2/s (sensitivity 80%, specificity 75.9%, p = .001). Multivariate regression analysis revealed that a high 5th percentile of ADC (> 1.078× 10-3 mm2/s; odds ratio [OR] = 10.494, p = .016), small tumour size (≤ 2 cm; OR = 12.692, p = .008) and low Ki-67 status (< 14%; OR = 10.879, p = .046) were significantly associated with low-risk DCIS. CONCLUSIONS: Assessment with whole-lesion histogram analysis of the ADC could be helpful for identifying patients with low-risk DCIS. KEY POINTS: • Whole-lesion histogram ADC metrics could be helpful for differentiating low-risk from non-low-risk DCIS. • A high 5th percentile ADC was a significant factor associated with low-risk DCIS. • Risk stratification of DCIS is important for their management.

BACKGROUND: As both intravoxel incoherent motion (IVIM) modeling and dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) provide perfusion parameters, IVIM-derived perfusion parameters might be expected to correlate with the kinetic features from DCE-MRI. PURPOSE: To investigate the association between IVIM parameters and prognostic factors and to evaluate the correlation between IVIM parameters and kinetic features in invasive breast cancer patients using computer-aided diagnosis (CAD). STUDY TYPE: Retrospective. POPULATION: Eighty-five patients (invasive cancers; mean size, 1.8 cm; range, 0.8-4.8 cm) who underwent diffusion-weighted imaging with 12 b-values (0-1000 s/mm2 ). FIELD STRENGTH/SEQUENCE: 3.0T MRI axial, IVIM-DWI epi-sequence, and DCE-MRI. ASSESSMENT: Two radiologists measured the apparent diffusion coefficient (ADC), diffusion coefficient, pseudodiffusion coefficient, and perfusion fraction (f) using IVIM modeling. Kinetic features such as peak enhancement and early and delayed enhancement profiles were acquired using CAD. STATISTICAL TESTS: The correlation between the IVIM parameters and kinetic features and the association between the IVIM parameters and prognostic factors were investigated using Mann-Whitney test and Spearman correlation test. RESULTS: There were no significant associations between IVIM parameters and prognostic factors. When IVIM parameters were correlated with kinetic features by CAD, both the ADC and f values showed correlations with delayed enhancement profiles. The ADC values were lower in tumors with lower persistent components (P = 0.013) and higher washout components (P = 0.045) and showed a positive correlation with persistent proportion (Spearman's rho (r) = 0.222, P = 0.041). The f value was higher in tumors with higher persistent components (P = 0.021) and showed a positive correlation with persistent proportion (r = 0.227, P = 0.029). DATA CONCLUSION: This analysis revealed that IVIM-derived ADC and f values showed correlations with kinetic features at the delayed phase as assessed by CAD. These results indicate the potential of IVIM imaging biomarkers to provide information on the biological and kinetic properties of breast cancers without a contrast agent. PURPOSE: To investigate relationships between intravoxel incoherent motion (IVIM) metrics of invasive ductal carcinoma of the breast and prognostic factors. MATERIALS AND METHODS: This retrospective study included a total of 82 masses from 72 patients who underwent 3T breast magnetic resonance imaging (MRI) and diffusion-weighted imaging. IVIM metrics of tissue diffusivity (Dslow ), pseudodiffusivity (Dfast ), and perfusion fraction (Dpf ) were measured using histogram analysis for whole lesion volumes. We evaluated relationships between IVIM metrics and prognostic factors or subtypes of invasive ductal carcinoma. RESULTS: The Dslow 50th , 75th , and 90th percentile metrics were reduced in the estrogen receptor (ER)-positive group compared with the ER-negative group (individually, P = 0.044, 0.024, 0.045). Additionally, the Dslow 75th percentile value was a significant differentiator of histologic grade, tumor subtype, and Ki-67 grouping (individually, P = 0.024, 0.049, 0.016). The Dpf mean, 75th , 90th percentile, skewness, and kurtosis metrics were correlated with the size of ductal carcinoma in situ (DCIS) component (individually, P = 0.034, 0.032, 0.027, 0.013, 0.013). CONCLUSION: The Dslow metrics differentiated between ER-positive and -negative groups, and the Dpf metrics were associated with the size of the DCIS component. LEVEL OF EVIDENCE: 4 J. MAGN. RESON. PURPOSE: To evaluate apparent diffusion coefficient (ADC) histogram parameters that show correlations with prognostic factors and subtypes of breast cancer. MATERIALS AND METHODS: At 3.0T, various ADC histogram parameters were calculated including the entire tumor volume in 173 invasive ductal carcinomas: the minimum, 10th percentile, mean, median, 90th percentile, and maximum. ADC parameters were correlated with prognostic factors and subtype. RESULTS: The mean ADCmedian value was significantly higher in the group with lymph node metastasis, HER2 positivity, and a Ki-67 value <14% than in the group with negativity for lymph node metastasis, HER2 negativity, and a Ki-67 value ≥14% (0.907, 0.978, and 0.941 vs. 0.735, 0.778, and 0.761 × 10(-3) mm(2) /s, respectively) (P < 0.01). There was no significant correlation between ADCmedian and tumor size, histologic grade, estrogen receptor expression, and progesterone receptor expression (P = 0.272, 0.113, 0.261, and 0.181, respectively). For most ADC parameters except for ADCmin , the mean of variable ADC parameters of HER2-positive, luminal A, luminal B-HER2(+), triple-negative, and luminal B-HER2(-) diseases were arranged in descending order (1.175, 0.936, 0.863, 0.811, and 0.665 × 10(-3) mm(2) /s in ADCmedian , respectively) with statistical significant difference (P < 0.001). In multivariate analysis, histologic grade, the Ki-67 index, and HER2 expression were statistically significant explanatory prognostic factors for ADCmedian and the Ki-67 index had the most robust effects on ADC parameters (standardized coefficient = -0.317). CONCLUSION: Various ADC parameters were correlated with prognostic factors and subtype, except for ADCmin . HER2 positivity showed high ADC values and high Ki-67 index revealed low ADC values.

OBJECTIVE: The purpose of this study was to compare the image quality of standard single-shot echo-planar imaging (ss-EPI) and that of readout-segmented EPI (rs-EPI) in patients with breast cancer. MATERIALS AND METHODS: Seventy-one patients with 74 breast cancers underwent both ss-EPI and rs-EPI. For qualitative comparison of image quality, three readers independently assessed the two sets of diffusion-weighted (DW) images. To evaluate geometric distortion, a comparison was made between lesion lengths derived from contrast enhanced MR (CE-MR) images and those obtained from the corresponding DW images. For assessment of image parameters, signal-to-noise ratio (SNR), lesion contrast, and contrast-to-noise ratio (CNR) were calculated. RESULTS: The rs-EPI was superior to ss-EPI in most criteria regarding the qualitative image quality. Anatomical structure distinction, delineation of the lesion, ghosting artifact, and overall image quality were significantly better in rs-EPI. Regarding the geometric distortion, lesion length on ss-EPI was significantly different from that of CE-MR, whereas there were no significant differences between CE-MR and rs-EPI. The rs-EPI was superior to ss-EPI in SNR and CNR. CONCLUSION: Readout-segmented EPI is superior to ss-EPI in the aspect of image quality in DW MR imaging of the breast.
